DS has been on 250mg/day of azith for about 3 months and we've had a lot of ups and downs. I've seen tremendous results at times, but things just aren't getting consistently better and he's in a downward spiral now. His ped has agreed to switch him to 2000mg/day of Augmentin.

 

I had such high hopes for azith, but we do live in Massachusetts and according to what I read by Swedo there are strep strains in the north east that are resistant to azith. Maybe that's the issue. I thought about asking to raise the dose of azith, but decided to try the augmentin. Has anyone else made this switch? Do you think it would have been better to try a higher dose of azith first?

 

PANDAS makes me constantly second guess myself. Aghh!

 

I am also working on getting an appt with Dr. Robert Fuhlbrigge at Children's Hospital in Boston, but he won't have an appt til February.

Hi, my ds has been on zith a while. Twice a week from October to Thanksgiving, then went to daily 500 mg for five days, then back down to 250. I'm in New York. I don't see much happening there either, if anything he's been worse, maybe three good days in last two months. Maybe Keflex is next, before Augmentin? Don't think higher dose of Zith should have been necessary, based on what I've learned.

 

Any thoughts on T and A (no jokes please)? We're going to move ahead on that in the next month or so.

Monarchcat...how much does your son weigh?

My ds is 80lbs and taking Augmentin 875 twice daily (per Dr. Latimer)... it seems to me you probably need a higher dose than what your doctor is recommending.

We live in NH an just switched from Augmentin 2000 XR (on for 7 weeks) to Azith. He was only on that for 4 days because ENT switched to Clindoamycin after his tonsillectomy yesterday. ENT doesn't like Azith. Not sure why. We thought we would switch back after 10 days of clindomycin. What does Dr. Swedo recommend for the North East?

my son just had T&A yesterday. I'll keep you posted. ENT did say they had a lot of "gravel" and white puss in there.

IMO a 95 pounder should have been on 500mg/day of Azith. Our dd was only 43 pounds when she started at 250mg/day azith (she's 54 pounds now).

 

But, since you are in the East Coast, Augmentin might also be worthwhile.

Is Dr.fulbrigge a neurologist at Childrens ? I was thinking about trying to make an appointment with a new doctor. I just don't want to go the route of a neurologist again. We had been seeing one at CHildrens. He was not good. I am curious to know the name of the neurologist Beth Maloney had seen at Childrens that was not helpful. I f I recall she did not name him in the book. We live in Mass also. My Ds is 7 and things are getting worse again and not better. Mood swings and explosiveness as well as motor and some facial tics. I am going to ask for augmentin from the ped. tomorrow. Can I ask where the information from Swedo on strains in the north east might be found. I just want to have all the answers for the ped. She is not a firm believer of Pandas. And when I bring Ds in he just doesn't look too bad. He presents well in the office but to live with it so crazy as you all know too well.

Dr. Fuhlbrigge is a rheumatologist. I know there are a couple of people here who have liked him and he's treated their PANDAS kids. He lists PANDAS as one of his specialties on the children's hospital web site. I *think* I remember reading something that he doesn't usually like the idea of prophylactic antibiotics, but I can't be sure.

 

I had a phone consultation with Dr. Latimer originally, but like others here, I can't get in touch with her now. I need a local doctor that can understand this and treat it. I thought about making the drive to see Dr. Trifiletti, but wanted to try Boston first since that's closest.

 

I'll look for the article that mentioned the resistant strains of strep. I know others have mentioned it here, as well, so maybe someone else will chime in.

 

I had such high hopes for the azith, and perhaps it was just that the dose was too low. We just started the augmentin so we'll try that for a month and then maybe go back to the azith at a higher dose if needed, I guess.

Thanks for the info. I am going to ask the ped. for augmentin today. Wish me luck. She does not like the idea of Abx without a positive strep test. Ds has not had a positive strep test since the big original episode. Although I know he is affflicted with the Pandas.

You can email her at info@savingsammy.net and ask her. She's very good about responding to emails. I think she couldn't name that doctor in the book for legal reasons, but I'm sure she'd have no problem speaking privately about her experiences, as well as feedback she's received from other parents since then.

Don't forget Keflex as an option- my instinct is to try that first before Augmentin. So if you go thru Augmentin and no dice, try Keflex next before thinking about switching back. Several members here have suggested it.

Had a pretty good evening last night. No apparent reason.

We just got back from seeing Dr. Trifiletti and when we went my dd was already on 2000mg of Augmentin XR per our family doctor. He added 500mg of Azithromycin per day. We'll start that tonight and see how it goes.

do you mean that you're going to give the 2000 mg. Augmentin AND the 500 azithromycin at the same time? or do you mean this is a switch?
