Son had no signs of Strep and throat and 72 hour tests were negative, but ASO was 1726 and D-nase was 1920 . I know that ASO should be around 200 but what should D-nase be for a 10 year old? He has now been on Antibiotics Aug 1000 XR for 30 days and his ASO is 1470 and D-Nase still 1920. He seems to be doing much better, but how? His titers are not that improved and I think they sound extremely high! If his numbers are so high then how can he be doing better? And..why are they still high? We have been phone conferencing with our dr. and Dr. Nicoliates. Another call in the morning. Praying hard!

In some kids, titers fall quickly, in others they fall more slowly....and some PANDAS kids don't get elevated ASO anti-dnase b at all! Remember, PANDAS isn't a reaction to ASO/anti-dnse b, so it is entirely possibly for a child with high titers to be improving (esp. if they are falling).

Just as a followon comment. PANDAS isn't a reaction to ASO or Anti-DNAse B. The high rates just indicate that he had a prior infection. For a healthy immune system it takes about 4 weeks for the ASO level to return to "normal" and about 8 weeks for Anti-DNAseB to return -- but in many children, and they don't know why, the fall is much much slower. Rejoice in the improvement, don't worry too much about the numbers.

ds has no aso or antidnase....but he's had strep 8-12 times in his life.....

current tests show nothing....cam was 105

myco p igg about 320 pos ds score 631

we did clarith...maybe some change???

when we added augmentin.w/i 24 hours i felt like i was really seeing something but didn't want to get my hopes up...w/i 6 days he was 50% + better..

when then had some allergic reacation....going to delv...further into lymes....

But do you think his positve reaction to aug was that are really fighting strep????/

What are your thoughts.....t

Really hard to say. Augmentin helps slow the growth of gram positive bacteria. It usually doesn't do very much for mycoplasma pneumonia -- but perhaps you cleared that out with the clarith. Hard to say why you got a good effect but glad you did.

I agree...hard to say. Assuming an allergy to Augmentin, how about trying Clindamycin or a longer/higher dose course of Azith?

since stopping aug we have been backsliding and things are not so good right now.

You all are amazing and such a wealth of information! I wonder if Pandas often hits our extra bright kids or are they just being discovered as having PANDAS because their parents are bright and go looking for answers!!! THANK YOU ALL!

Dr. K told me yesterday that the #'s really don't make a difference. You can have really high titers with little symptoms or really low titers with a lot of symptoms. I think once PANDAS has been diagnosed it becomes the amount and severity of symptoms to watch. I think the titers and cultures are helpful in original diagnosis.
