New Flare...Could it be her tooth?

DD is diagnosed with PANDAS with main infection being Myco P. The past few days she seems to be in the midst of a flare. I've been racking my brain trying to figure out her trigger... She could have been exposed to a new illness but who knows... School also started and that also could be a trigger but Monday that a tooth that has been vacant for almost a year is finally breaking through.

 

BG- In September DD had a crown put over a cavity that fell out. This resulted in a 5 day flare... Worst we had seen... We didn't realize they were related until we had talked with our PANDAS Dr. Then it made since.

 

So my question is am I just grasping for answers or could our setback be bc of the new tooth... Any one experience this?

I think it could be her tooth. Anytime my child goes to dentist or his gums bleed from flossing, etc it causes a flair. Usually only lasts a few days but he is on abx as well so that probably helps shorten the duration. If she is not already on abx, try get her some so that it will help the body fight any foreign bacteria that may be entering her bloodstream through the hole where the tooth is coming in.

My son was pretty free of his worst rages for more than a month, than the day a tooth fell out, he had a major rage.

I took these notes from the emails people sent from the PANDAS conference:

xxx mentioned that a flare can be brought in by a simple cold or
virus, even if the child doesn't seem to contract it. Another cause of flares
are loose teeth or teeth coming in. Any source of inflammation in the body can
set off a flare. She said not to panic. These types if flares general last only
8 days and they will return to their baseline soon.

Our ped explained the tooth connection like this: Strep is everywhere (unfortunately) - and loves to hang out in the mouth/on the teeth. When a tooth falls out/cuts through/cleaning/dental work, etc... there is an opportunity for the bacteria to enter the blood stream - triggering a flare. He said that is one of the reasons dentists will prescribe abx for rheumatic fever patients prior to dental work.

Thanks for the replies!!! We are on Azithromyicin and it has been better than her September flare (when she got a crown). So this is probably day 4 or 5.... Hoping it stops soon.

 

My biggest concern is she has about 12 teeth to lose.... Any ideas on how to stop this each time she loses a tooth. I guess getting her infection numbers today.

 

We actually ran labs today so it will be interesting to see what it comes back as...

Absolutely! Happened to us all the time. What made things worse...is my daughter would need the tooth to come out right away and rock it back and forth, releasing all that yucky bacteria. Worse thing you could do - drove me crazy. Thank goodness she only has 1 or 2 baby teeth left.

 

Oh, and have issues with dental cleanings too. I had to tell the dental hygienist to do the minimal amount of cleaning necessary. During our last visit they switched hygienist without me realizing. It was the most painful cleaning she had ever had. By the time we got to the car she was in a flair. I was so mad.

Meant to comment on this post earlier .... We are right there with you. Dd7 is in the midst of losing all kind of teeth - three in the last few months. Another one is loose now. Well she has been flaring since mid November. :-0. She is on a combo of 3 abx (she is Lyme/Bart also) and we were convinced she had picked up some kind of infection. All her lab numbers have been going down since September which is great! Trying to figure out also if the teeth are generating this flare.

Wow, I'm so glad I found this thread. I was just posting on my thread that DS has had a setback and I thought he may have picked up a virus. But a couple days ago, I noticed he was cutting a new tooth on the top but I didn't make any connection between that and his flare.

 

His baseline is a certain frequency/severity of tics but these past two days it's like he's been going insane. Flailing windmilling arms, walking beside him after school it's like I need a shield to prevent my side from getting beat up. He's also started screaming a bit.

 

mommy2mcl, how is DD now? It's been more than a week since you first posted. Have things calmed down? Did you give any ibuprofen?

Yes we gave ibuprofen and it helped. Her tooth cut through prob Sunday or Monday a week ago. She displayed tics we hadn't seen in months, emotional liability, OCD and compulsions. She is on Azithromicin for MycoP. I thought we could handle it and I made the mistake of not calling our specialist until Friday when we saw little improvement. When I called they wanted to stop Zithro and start Augmentin. But by Friday I think she was doing better and putting her back on Augmentin actually caused regression because the tooth was better. Yesterday and today she has been so much better after we restarted Zithro ! WE also let her soak in Epsom Salt. I have read that it has tons of calming properties and figure it can't hurt (I hope)

 

So I am not sure what medicine your DS is on but I would definitely call if you can. My worry is we have only lost 8-10 teeth... How do I deal with this every time.
