marilina Posted February 12, 2010 Report Posted February 12, 2010 few days ago I wrote on this forum: My daughter is 17 years old. the first suspected PANDAS episode dating nine years old. Last year it started to deteriorate. When I wrote the last time I reported that the antibiotic was working, but at the same time have the appearance of petechiae on the legs: in the past had already happened a couple of times but had lasted a short time and the cause was most likely excretions of heavy metals. This time I'm afraid to give her more abx because one of the augmentin side effects may well be a problem with the number of platelets that could fall. obsessions are getting worse. I do not know what to do.

We all know how you are feeling ,call the md and tell him whats going on.There could be a different antibiotic she could take.Has she tried Zith? Its good that she responded at all. Melanie
Suzan Posted February 12, 2010 Report Posted February 12, 2010 I am sorry this is happening! Do you believe your doctor will help? Like an increased antibiotic dose (if possible)? Have you discussed whether IVIG would be a possibility for her? Susan

thereishope Posted February 12, 2010 Report Posted February 12, 2010 Can you break down for us what meds she is taking and for how long she has been on them? If you see a big worsening of symtoms for days w/ no improvement resurfacing, I'd probably try to change the meds or go in for a strep test while reminding your doctor of the possibility of a false negative because she is currently taking abx. When you see a worsening w/o infection, you have to look at it from a lot of angles. Like already mentioned, can she have a virus, allergies, high stress (esp during the early stages of recovery), is anyone else in the family sick? You can go ahead and get family members tested for strep again since some kids do react full force to exposure only. I'll have to google "petechiae". I haven't heard of that before.
marilina Posted February 12, 2010 Author Report Posted February 12, 2010 here Vichie are some pictures: http://www.wellsphere.com/wellpage/severe-leg-bruising this is about Augmentin side effects: http://www.drugs.com/sfx/augmentin-side-effects.html All medicines may cause side effects, but many people have no, or minor side effects. Check with your doctor if any of these most COMMON side effects persist or become bothersome when using Augmentin: Diarrhea; nausea; vomiting. Seek medical attention right away if any of these SEVERE side effects occur when using Augmentin: Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); bloody stools; confusion; dark urine; fever, chills, or persistent sore throat; red, swollen, blistered, or peeling skin; seizures; severe diarrhea; stomach pain or cramps; unusual bruising or bleeding; vaginal discharge or irritation; yellowing of the skin or eyes. thank you all thank you Karen: it seems that for your son petechiae and abx assumption is not correlated also...
MomWithOCDSon Posted February 12, 2010 Report Posted February 12, 2010 My son is also on Augmentin . . . has been since October. We haven't seen any of the bruising at all, however, either before or after the abx. As for side effects, everyone seems to respond differently. Other than the petechiae, have you seen any of the potential side effects in your daughter? I've heard others report nausea and/or vomiting with Augmentin, but that has not really been our experience. While on regular Augmentin, DS12 reported some nausea and had some diarrhea, though we were never entirely sure whether it was the abx or his OCD-oriented "nervous stomach." When we switched to Augmentin XR, pretty much all of the side effects went away; no nausea, no diarrhea. We also pump him with probiotics, though. Have you tried one of the time-release abx, like Augmentin XR or Moxatag? Strictly anecdotally, those seem to be a little easier on the gut. If she is experiencing any additional side effects, Azith may be a viable answer, since that also seems to work for a lot of these kids.
colleenrn Posted February 12, 2010 Report Posted February 12, 2010 Can you describe what the petechiae looks like? Is it only on the legs? Is there any possibility it could be a strep related rash? My children, when they are exposed to strep or have strep, develop a rash that could be looked at as petechiae, but I know it is not. Their rashes are not the "typical" strep/scarlet fever rash, but one thing I have learned in the years of dealing with PANDAS, my children don't get "typical" symptoms. If it is related to the Augmentin, can you switch to Zithromax? Colleen

Colleen yes she has them only on the legs petechiae may be related with low levels of platelet.......Unfortunately this problem can be triggered by the use of antibiotics. this is my doubt, the spots have appeared , I'm sure, days earlier, but during the use of the antibiotic increased. I'm waiting for the results of blood tests: to rule out problems with the liver and platelets
Kayanne Posted February 12, 2010 Report Posted February 12, 2010 http://en.wikipedia.org/wiki/Petechia Wikipedia also lists scarlet fever as an associated illness with petechiae Scarlet fever is a strep infection that also has a rash...I would push for a change to azithromycin I could be related to the strep.
