DS (8) has been on 250mg/day Azith for about a month and a half (5 days of 500mg/day early on) and has been tolerating it well. He is also starting his 4th week of prednisone, tapering this week with 10mg/every other day (from 20mg/day last week). Last night he complained of an itchy rash on his chest and back. There are tiny, individual bumps, some with tiny whiteheads on them. Seems a bit worse today than last night. I still gave him his azith this morning because it seems to me he's had these little bumps once before, though not as bad, before he was on the azith.

 

The other thing is that he has been ticcing like mad the last couple days. Constant neck tics that we haven't seen in months, along with PANDASy irritability, moodiness, paranoia, and inflexible thinking. And this is while on prednisone!

 

Of course, this stuff always happens on a weekend so I'm not sure if I need to put an emergency cal into the ped's office. *sigh*

 

Anyone have any experience w/ a rash while on the azith? I know it can be a sign of an allergic reaction, so I'm concerned.. just don't know how concerned I need to be yet.

Monarchcat, there are so many things it could be. Since it is itchy and pimply (and ds has been on a pretty high dose of prednisone), it is unlikely contact dermatitis and sounds more like a viral rash. Definitely worth going in to see a dermatologist. Has the rash spread?

It has spread a bit, down his tummy and back, but still seems pretty mild. My guess was viral rash as well, and he did just get over a cold/cough type virus so maybe it's all related. My worry, of course, is an allergic reaction to the azith, but I would think it would get much worse after today's dose if that were the case?

did the doc state that tics could increase while on the prednisone. Since Char is noting this too, I don't know what to make of it. I don't know what the future holds for us as far as treatment, but I'm scared to death of doing things that may increase my son's symptoms, and so tend to shy away from them. I always like to see what others experiences are, so needless to say, I'm not feeling hopeful after hearing of these two increases today in ticcing while starting the pred.

 

Could others comment?

I took him to the Dr. this morning because I wanted to be sure it wasn't an allergic reaction to the azith. Dr. said it wasn't and that he thinks it is either folliculitis (infection of the hair follicles causing little pimple-like eruptions) or a viral rash. Either of these is plausible because I guess you can be more prone to folliculitis after being on antibiotics or steroids (according to what I read when I got home).

 

My guess, though, is that it is viral rash. He had a cold/cough last week, and though he's pretty much fine now, I wonder if it's related. His PANDAS symptoms have ramped up big-time the last few days, coinciding with this rash, so I have to think there's a relationship.

 

To be honest, I haven't seen a change since being on the steroid, and obviously things are now even worse due to whatever virus, etc. we are dealing with.

 

Faith, I don't think you have to worry about symptoms INCREASING on prednisone. I think in our case the pred didn't necessarily help the tics (at least not yet) and the increase is due to a virus, not the pred.

Maybe a yeast rash? Our doc is not big on long term abx nut I have been pressing him regardless he mentioned that some people get a yeast rash from the antibiotics. I don't know what this looks like but have it checked out.
