My daughter has a purple lump on her leg (shin area) that seems to be getting bigger. It looks like a bruise but sort of swollen too. You can feel the heat radiating from it. It hurts her and is slightly itchy at times. At first I thought it was just a bruise, then I thought maybe a spider bite...now I'm not so sure. I've noticed it for about a week, but she told me this evening it's been there much longer, just not as swollen and purple. Any ideas what it could be?

I think it's Erythema nodosum. Anyone have any experience with this?

 

Can you get the from Strep or Rheumatic fever? I thought that is what I remember hearing but not sure. What about Bartonella? I am sorry, I don't have any experience with it specifically.

From what I read on the internet, it can be associated with streptococcus infections, EBV, inflammatory bowel disease. It can also be from cat scratch fever (bartonella henselae). Other causes are hepatitis B,tuberculosis,salmonella enteritidis,leukemia & lymphoma. And, I think you can also get them from non-infectious causes as well.

 

My daughter currently has elevated D-Nase and a normal ASO (but increased from previous numbers). She also has elevated liver enzymes- AST-282,145; ALT 354,161. Waiting for new results drawn last week. I'll guess we'll see what the doc says. thx

 

edit- just received partial results of labs. Dd's AST is now 61, ALT 53...so going down. Her Total Protein & Globulin (Hepatic Panel) is now elevated.

So, not back on antibiotics yet? I have been thinking about you and wondering if your dd was still off antibiotics and wondering how her liver enzymes and d-nase are. I hope you don't mind if I compare notes with you.

 

I had posted on your thread on the PANDAS board about dnase rising. We just got labs back again and dnase is even higher than last post.

Taken every month and they have gone like this (late in the month): July 165, August 487, Sept 598, October 742,

 

ASO has gone up slightly but still barely below the upper range of normal. ALT just above normal.

 

I keep thinking he needs to be on different antibiotics and/or more frequently. Treating for Lyme, Bartonella and Babesia. That is the focus instead of strep. But I really keep wondering if he needs augmentin for strep - when he went on in Dec - his dnase finally went down in Jan., then he went off mid June and you his dnase is elevated in mid july

 

To me behavior seems PANDAS-like for him no illness symptoms behaviorial stuff (not as extreme but not as good either) that led us down the PANDAS trail in the first place in 2010 before pursuing Lyme in 2011. Defiance, random acts of throwing, ripping up objects some aggression (not a fluently verbal kid but that is actually improving). I wish their weren't so many variables

 

Philamom - Let us know what your doctor says for your DD. Thinking about you guys.

 

Also input of anyone who has insight of the interplay of treatment for Lyme and coinfections and strep. I have call into LLMD

aidan's mom- Thank you. We have our next appt with LLMD in a few weeks - I'll let you know what he thinks. I should get the ASO/DNase results any day. Tomorrow I have an appt to have her leg checked out with our local doctor. By late afternoon it's really hurting her.

 

Are you still pulsing abx?

Yes we are still pulsing. I talked to LLMD last night on the phone and right now we are going to stick with current antibiotic/babesia treatment schedule. I think I am conflicted at viewing this from a PANDAS treatment point of view vs. a Lyme/co point of view.

 

Let us know what the Dr. says about the purple bump.

Local doctor thinks it's a staph (cellulitis) infection. He feels she needs an antibiotic and prescribed Keflex for 10 days. Thought it would be easiest on the liver. He also feels dd may still have strep and the keflex should address both. I just need to run it by our llmd first.

Just got back recent ASO/DNASE:

 

ASO 98 (normal but increasing): May-35, Aug-48, Sept-61, Oct-98

DNASE 477 (High but decreasing): May-50, Sept-663, Oct-477

 

I still feel this lump may be strep related. Hopefully, the Keflex helps...so far(only 4 doses), nothing.
