Ozimum Posted January 14, 2011 Report Posted January 14, 2011 Hi, My ds16 is to have two impacted wisdom teeth out under general anesthetic next week...unfortunately they have to come out. Our DAN dr has recommended he takes buffered Vit C for a week before and a week after surgery, which we've started. (Any suggestions as to dosage?) He has also recommended he be put on Erythromycin (from which Zithro is derived) and the surgeon has no problem with that. (Ds is being treated for leaky gut so we want to avoid the big guns like Augmentin in order to maintain the good work already done.) Meanwhile I have everything crossed and I'm having palpitations just thinking about it! Anyone been here? I would be so very grateful for any advice. THANKS!
thereishope Posted January 14, 2011 Report Posted January 14, 2011 (edited) Some, not all, PANDAS kids do have setbacks post dental work. I agree with antibiotics. If you start them a few days before and then at least a week+ after. Ask the dentist about Ibuprofen. Not sure if you can take it with dental surgery as it could thin blood. Hopefully, those who have been there will chime in. My son is only 7. He does fine with cleanings, loose teeth, falling out teeth, and did fine with anesthsia with t and a surgery. DS been ok with the orthodontist for a while now, but with this op I'm terrified of the potential for another strep infection (or anything else for that matter). I have to trust that all the work we've done with his DAN dr will pay off, and he'll coast it in, but I do want to head off as much as I can and be ready should things go pear shaped. Good point about the Ibuprofen - I'll check on that. Thanks!

The surgery needs to be done or leaving it be could cause pain or leaving it be can cause an exacerbation as the wisdom teeth try to push through even more. Again, not all kids have reactions. Does the surgeon know he has PANDAS? He'll wear a mask during the procedure and if you have antibiotics, hopefully you'll have your bases covered. Has he taken any stronger pain meds before? They may give him Vicodin.That's what I was hoping someone would chime in about...if there are any pain meds to be on guard about. You may be able to ward off pain with a combo on Tylenol and Iburprofen (dentist approved), but you may need something stronger. Edited January 15, 2011 by Vickie
Ozimum Posted January 15, 2011 Author Report Posted January 15, 2011 DS has had two ops before - appendectomy and knee, but before PANDAS. He handled both well and I was not this much of a worry wart then! I've not mentioned PANDAS to the surgeon - I got as far as gut issues and I could see that was enough for her. I'm not sure that Vicodin is available here...I don't recognise any of its trade names. I'm expecting he'll be given a paracetemol type pain killer such as Panadol (which I think is like Tylenol) or Panadeine, possibly even Panadeine Forte. Whaddya reckon? If I get myself in a right tizz over this there won't be a problem???!!

LNN Posted January 15, 2011 Report Posted January 15, 2011 I'm sure the abx will be more helpful, but as an added measure, you can ask the oral surgeon for a mouth rinse called Peridex. It's an antibacterial rinse that my DDS gives when I've had root canals, to keep the area a little cleaner (if that's possible in the mouth). My son uses it for a few days just before and after losing a tooth. Not sure how effective it is, but it helps you feel like you're doing all you can. Take a deep breath. Your goal is ling term health and if removing the teeth is part of that goal, then remind yourself you're doing the right thing, even at the risk of any temporary set back. Hopefully, the tizzy you're in now will prove to be much ado about nothing.

And, reading the threads and links on anesthesia made me think we may have been really lucky with the last two anesthetists! Good info there. Ever feel amazed just how much info we find ourselves getting across because we have such vulnerable children? Talk about getting a PhD in life! Thanks so much for your input! There were a couple of good articles. can you tell why you say to stay away from laughing gas? my dd is being sent to a dentist for work to be done using laughing gas because of her anxiety over the procedure. they have suggested sedation too.
