TU There is mucosal thickening of the left maxillary sinus and of the left ethmoid air cells, The remaining paranasal sinuses are within normal limits. IMPRESSION: Sinus inflammatory changes in the maxillary sinus and ethmoid air cells otherwise unremarkable MRI examination the brain with no abnormal enhancement or structural abnormality,

Either way, inflammation can cause a breech in the blood-brain barrier and result in PITAND exacerbation. If you have mucosal thickening in the maxillary and ethmoid sinus, then there also is likely reduced drainage, which sets up a warm and moist environment and high risk of bacteria forming to make an infection. The ethmoid sinus is between the nose and the eye and it drains into the maxillary sinus which is by the front of the cheek. MRIs do not give a lot of detail about sinuses (CAT scans give more detail but also have more radiation). If you are seeing mucosal thickening in an MRI there may be a small infection that would be picked up on CT scan. Hard to say. DO NOT ignore this. Get to a good ENT or allergist and get treatment. Dr. B told me that for my son with PANDAS with sinus infections we needed to: a. Treat and remove any infection b. Aggressively treat allergies and inflammation c. Use IVIG to reset the system if needed after allergy treatments. PM me if you want more details. Best wishes
