We went to the Dr. a week ago and he agreed that PANDAs seems like a reasonable diagnosis. He gave us a prescription for 845mg Augmentin (DS is 14 and 170 lb) and a tapered prednisone burst for 18 days. He also recommended we get the Cunningham test and prescribed a boatload of immunological tests. Poor little kid really worked himself up over the blood tests--first he almost fainted and then he threw up. Later on, though, he agreed that the test wasn't that bad. Go figure.

 

I've started the Augmentin (we're on Day 2.) No effects so far, except that he seemed slightly more reasonable when I told him he couldn't touch the screen on my laptop. He still did it though.... :)

 

I haven't been able to give him the Prednisone yet because I am still too worried about it setting off a tic storm. I wanted to give it to him on the weekend so I could watch him. Sadly, it appears that I won't have that kind of weekend this week. Does anyone know if it would mess up the whole dosage schedule if I gave him one 10 mg pred. pill rather than starting off at 4 and then tapering down? I'd just like to see how much of a train wreck this will be.

I wouldn't think it would hurt anything to give 10mg to see how he responds,(I'm no doctor, you could call and ask them) but it won't offer the reduction in inflamation that 40mg does either and it can take a while to kick in. Has he been diagnosed with Tourettes? Has he had PANDAS for long? Have you tried giving him Advil to see if it makes a difference? My dd gets a good amount of relief from Advil. (not tylenol) It reduces inflammation enough to offer some relief.

Yes, DS has a Tourettes diagnosis, and likely has had PANDAS since he was 5--so 9 years. Advil seems to help his mood and OCD a lot, one reason I am entertaining the possibility of PANDAS. I'm going to try the 10 mg, and then if he doesn't go into a tic storm (this has happened when I gave him some suppliments like Acetyl L Carnitine and taurine), I will go ahead with the rest of the prescription as written. Thanks for the response, and I'll keep you posted.

I went ahead and gave him the prednisone (all 40 mg as written) He seems a bit more edgy and OCDish, but no tick storm today (we'll see about tomorrow). In general the Augmentin seems to have made him a lot angrier so far (we are 4 days in) and he already was way too angry. However, I'm going to let it ride for at least a week or two and see if this is die off or some adjustment routine. Otherwise, my plan is to ask for another abx. Has anyone had an initial anger reaction to augmentin that later straightened out?
