Hi,

Just have a quick question Anthony's DNASE-AB level went from 960 until 1360 on Augmentin. He is still having some tics so my question is do I push to get on Zithromax or I do I leave things the way they are?? Need so help with this one.

I'm not an expert or a doctor...but I would think that a rising titer means that the infection is still present.

 

AntiD nase titers are not the antibodies responsible for PANDAS, but still a rise may indicate strep is still present? Anti D nase is more a long-term titer, and I'm not sure really what it rising means

 

I am getting to the point (personally for my family) that I just don't trust the penicillins against strep--even as a prophylaxis.

 

Cephalosporins have worked better for us--this round of strep in our house they did the trick. However, I do think the strep strain that hit last year was a bad one.

 

Some doctors are resistant to zithromax (usually because of geography) because of resistant strains of strep to it. My pediatrician is much more comfortable with a cephalosporin instead. So, if you hit that reluctance...try for keflex or omnicef. On Feb 24, I took my PANDAS daughter to the pediatrician because she was showing some PANDAS behaviors and my other 3 kids were positive...She, of course was still negative...but the ped still put her on full strength of Omnicef. But, she told me to split the dose to 150 mg 2/x day....that makes sense to me...ya know keep the levels of the antibioic more level in the blood...

 

Well, 300 mg of omnicef is 50 mg higher than the treatment dose. Just get aggressive with the probiotics.

The 2 most popular antibiotics that work on here seem to to Augmentin and Zithromax. However, some have cited that Omnicef and even Keflex have been the mosts successful in their children. Sadly, it is a lot of trial and error and not every child is the same.

 

If he's been on Augementin for a bit and you still don't see improvement, I'd probably try to change abx.

One of people who did this study was the same author as that popular article about the amox and pen failure rate with strep...

Hello,

thanks for the info--I think my doc will change just can't get a hold of her. I have left 8 messages since Monday for Dr. Latimer she responded to a quick email I sent her on Monday and said we would talk Tuesday well I still cannot reach her any suggestions to help with that. She is good but this piece is really is frustrating. Andrea one of her girls at the front desk says she is getting so busy and I understand that but if you have a patient and you order labs and things come back out of whack you need to be available to talk about them. I can't call another dr. and say so and so ordered labs this is elevated or whatever and have them treat it. sorry to vent just so frustrated.

What is the dosage amount and your son's weight. Perhaps upping antibiotic until you can get a hold of Dr. L or 'see her in person' again. I noticed you are local to her. Have you seen an increase in TICS/behaviors with the rise in titers?

 

My friends son was struggling..... he is now on 250 m.g Augmentin and 250 m.g Azithromycin and really made a turn around in 48 hours (45 pounds). Our son is on also on 500 m.g. Azithromycin per day (45 pounds) and doing well. We hope to lower the dosage once the strep session is over.

Does your son still have tics? I did shoot one more email to Dr. Latimer she is going to put Anthony on 250mg Zithromax every day. So she finally called and did apology--all is good. thanks.

Our son has had '3 IVIG treatments' in conjunction with antibiotics. He has one mild TIC (actually not TICing at all today), irregular, mostly in evenings that looks like it will resolve, no OCD, some occasional mood liability.

 

Wishing you all the best in your son's recovery and so glad you were able switch antibiotics.
