I spoke the ped last week who said they were all "normal," and that he failed only 5/14 pnuemococcal titers. Now I am looking at the numbers myself, and I see that the IgA and IgM are indeed right in the middle of the normal range. The IgG subclasses 1-4, however, are all at the very very bottom of the range. Does this constitute a deficiency after all? Is it just me, or does my ped have a funny idea of normal? Do immunologists look at these numbers more critically? Also, on the pnuemococcal titers, it says antibody levels >1 demonstrate immunity... but by this standard he actually failed 8 of them, though it's true that he was only out of range, >.04, on 5 of them.
KeithandElizabeth Posted October 16, 2009 Report Posted October 16, 2009 I am also still trying to completely understand all of this.... So, our son failed 13 our to 14 of the S. Pneumo. Failed on our lab sheet by Quest said anything less than a 2. He also passed the IGG levels, but was at the very bottom of the barrel on IG4 and total IGG serum. Then he had 3 high Lymphocytes CD levels. Taking into consideration this blood work as well as his PANDAS symptoms is why our immunologist wanted to do 6 months of IVIG. Now, our daughter also failed 13 our of 14 of the S. Pneumo. She did have one level over a 3. She also passed the IGG levels, but at the bottom of the range for IGG total Serum. She was 833 and the range was 823 to 1900. But she does not show PANDA symptoms and she did not have high Lymphocytes. Hence, our doctor put her on a IGG prescribed supplement and wants her to avoid soy, dairy and wheat (yikes). So, I think that each treatment plan is based on each person individually. It is interesting though how each doctor also does things al little differently. Elizabeth
Debbie1 Posted October 16, 2009 Report Posted October 16, 2009 Bronxmom, Did your son have the pneumoccocal vaccine? My dd did not (she's 12). She failed several of the pneumoccocal titers but she passed the 8 main ones. I was told her results were normal becase she never had the vaccine. The only way to have the antibodies without the vaccine is to actually have contracted that strain of the virus. Dr. L. looked at our results and also said they were normal. Debbie

Do you know an Immunologist in your area whom you could mail the results to with a letter stating you would like to visit with them on the phone regarding whether the results are normal and follow up with an appt if they feel your child should be seen?

It's just funny that the pediatrician assured me over the phone that everything was totally totally normal on this report (with that tone in her voice that they reserve for crazy moms)-- I had to keep pressing them to send it to me-- then I finally get it and everything's low.

It sounds like Dr. Latimer will see the results, so you can get her opinion. If you're still not sure you're getting the right answers, talk through these results with an immunologist who is familiar with PANDAS, even if it's just a phone consult.
