kakrpa
Hi all, I know there's a symptom severity scale to help clinically dx PANS/PANDAS, but are there any guidelines/diagnostic criteria for blood results that dx PANS? For example: high coxsackie levels + high mycoplasma + dx OCD = PANS. Again, that's just an example - wondering if there are blood/lab based results that clinically determine PANS dx. Thank ya
pr40
you are forgetting one thing in your formula, sudden onset. when you do add that, you can go to symptom scale which will then serve your purpose. and you don't need the formula you are looking for -- if I understand your question correctly. sudden onset = infection. now, you do blood work to figure out which treatment you need. kakrpa
Thanks pr40, you're right...forgot about that. OCD onset was rapid (for a 22yo it happened a few weeks after what we're now thinking was pneumoniae). Labs show very high Coxsackie a and b levels, high mycoplasma, and low IgG. kakrpa
I'm working with a PANS specialist, and would like to show my results to my regular Dr and Psych Dr, but am afraid of being dismissed. Since already dx OCD, I'm afraid w/o more proof I'll just be dismissed.
pr40
you can use NIH, suggest that this is a new way of thinking about some cases of OCD. refer to Yale PANDAS study and Harvard which is opening PANDAS clinic at the MGH and Stanford which already has one. Mayzoo
Cunningham panel is the only test that has an indication level for PANDAS/PANS as likely, or not. You can use the search feature here and put "Cunningham Panel" in to do more research on what it tests for and what the outcomes are of testing. We opted not to do this test because kiddo had strep at onset and 13 of 14 (likely had all 14 but due to verbal delays, we were unsure) symptoms at that time. I was unwilling to spend the 1000 to 1200 dollars.
Mayzoo
PANDAS and PANS diagnoses are based upon defined clinical characteristics. The Cunningham Panel™ tests for autoantibodies directed against specific neurologic antigens and receptors, and measures the state of the stimulated immune system against HUMAN neuronal cell lines at a single point in time. The results from the Cunningham Panel™ are provided to the physician as an aid in their diagnosis of PANDAS or PANS. There are treatments that are known to affect the test values of this panel, which include: recent treatment by intravenous immunoglobulin (IVIG), plasmapheresis or plasma exchange, and steroid treatment. http://www.moleculera.com/testing/
kakrpa
You all are awesome - thank u so much. I did get back some additional lab work. The below are reported high. Thoughts? EBV Ab VCA, IgG = 54.8 (range 0-17.9) HHV 6 IgG antibodies = 7.78 (range 0-0.99) These results are in addition to previous lab (about a month ago) that revealed high coxackie a, b, and high mycoplasma p. Also low IgG. Edited January 27, 2015 by kakrpa
