antibiotics high dose vs lose dose

Son, 23, 130 lbs has been on 250 mg zithromax for 14 days awaiting labs for ivig. I have seen difference in movements Dr says is from syndenhams chorea. The movements has totally stopped in last week. OCD is still there.

 

I have asked Dr to give higher dose but he feels length of duration at low dose is more important than dosage itself.

 

Son is allergic to penicillin, augmentin and keflex so Zithromax is the choice.

 

Can you please give me your thoughts on dosage? What worked best for your kids and for how long?

My dd started at 250mg/day (but she was only 43 pounds at the time). She has been on that since 2008. Now she is 12 years old and 99 pounds. She is still on 250mg/day, but we have a rx to up it to 500mg/day if needed.

 

I think 500mg/day would be a better dose for a person who is 130 pounds...then go down to 250mg/day in 2-4 months if things are doing well (if you hit a plateau). But, be ready to increase back to 500mg if there are problems on 250mg.

I heard somewhere (probably here) and it makes sense to me, that lower doses of abx may cause abx resistance. If the dose isn't high enough, the weakest strains of bacteria are killed off, but the stronger ones survive and proliferate because they have less competition.
