Melanie-

After reading the article posted by PhillyPA, this looks rather significant. When the result is that far above normal range. The IVIG is one of the treatments the doc in the article used. VERY interesting. I hope this is an important & beneficial find for you. Could the GAD autoantibody be causing thyroid problems like it can with the pancreas? Dawn

This is a from quick search on significance related to Diabetes:

"GAD test detects the presence of antibodies to glutamic acid decarboxylase (GAD), which provides early evidence of autoimmune disease activity; its measurement has been shown to be useful in assisting the physician in the prediction, diagnosis, and management of patients with diabetes.

Type 1 diabetes, commonly referred to as insulin-dependent diabetes (IDDM), is caused by pancreatic beta-cell destruction that leads to an absolute insulin deficiency. The clinical onset of diabetes does not occur until 80% to 90% of these cells have been destroyed. Prior to clinical onset, type 1 diabetes is often characterized by circulating autoantibodies against a variety of islet cell antigens, including glutamic acid decarboxylase (GAD), tyrosine phosphatase (IA 2) and insulin. The autoimmune destruction of the insulin-producing pancreatic beta cells is thought to be the primary cause of type 1 diabetes.

Autoantibodies to GAD is found in type 1 diabetics at and prior to disease onset. These autoantibodies are generally more prevalent in younger onset patients. Because the risk of diabetes is increased with the presence of each additional autoantibody, the positive predictive value of the GAD antibody test is enhanced when measured in conjunction with IA 2 antibodies and insulin."

In addition to article posted on child that went nutty with elevated GAD- neuro symptoms- I also found references to increased GAD in Stiff Man Syndrome which is an extremely rare autoimmune disorder. Nobody wants to have that, either!! So...while I can't be that helpful... IF he does not have obvious signs of Diabetes (elevated glucose, frequent thirst, frequent urination, weight loss, blurred vision etc then it appears that there is evidence of some type of autoimmume process going on to cause levels this high. Unless, this is some type of known effect from some medications he is on???

Hopefully, docs can shed light on this finding with the return of normal office hours:) Keep us posted on what you find out. Fascinating, on some level. Just never know what is going on because so much is not tested on kids with these specialized tests. Then to have this type of reading.

Both my parents are diabetic type 2 and it runs horribly in my family. My dad had ITP and there are so many immune issues it's awful. On both sides there are. My son got blasted basically. PANDAS is autoimmune. Sadly my son is one of 3000 patients in our practice with it or actually diagnosed with it. I wish they would hurry up and excempt them from vaccines. His ped still wants him to have his middle school booster and I don't in a few years. Glad we have a few years to fight about it.
