Does anyone have any good ideas on how to document daily? When we go to the doctor they ask a lot of detailed information and sometimes I get confused myself on when behaviors started, how long they lasted etc... Just curious if anyone had any ideas on how to keep everything in sync.

I just wrote down bullets every day then wrote journal entries based on those when I found time. I kept a small piece of paper on my kitchen counter and when he did something negative, I jotted it down, when he did something good that showed progress, I jotted it down. Even if I looked back at my small notes a week later, it would help me then elaborate on it in a journal entry. Like you said, everything blurs together after awhile or you simply forget.

I have used the chart that Vickie posted, though I have to admit that sometimes it's a little tedious for my use everyday.

 

I've resorted to a fairly concise digital journal these days where I note changes in supplements or meds, dosages, etc. And then I make some very basic notes (up arrows, down arrows, etc.) for key behaviors and how they're trending: obsessive thoughts, rituals, anger, overall mood, sleep, etc.

 

So far (knock on wood), his doctors have been receptive.

I keep it brief and I put it on Imac calendar. It is coded by color, ie. post-infusion is blue, pre-infusion red. The calendar seems simple and my husband can add his own comments.

I created an excel spreadsheet based on the system Vickie posted (with personalized symptoms). I programmed it to graph and chart progress. It's been simple to use and the doctors have liked the reports it makes. If you PM me your email address, I'd be happy to share it with you
