Letter from Doc to School

I am trying to get a letter from our doctor confirming for the school that my son has PANDAS. This is part of getting an IEP in place. I'm going to draft the letter and ask my doctor to sign it. Does anyone have a letter that they would be willing to share? I want to make sure I have everything covered the first time!
