familyof4 Posted December 11, 2009 Report Posted December 11, 2009 Hi, Does anyone have a sample 504 plan that they could share with me? My son, 6, has missed a lot of school this year. Although these absences have all been covered by a doctors note we are getting "warning" letters from the school district. The last thing I want is to be hauled into truancy court. Won't having a 504 plan in place help give us a bit of "protection"? Additionally, his music teacher graded him extrmemly low with comments like "lack of effort" and "absences are impacting his grade". I am trying to set up a meeting with her. At his concert last night it was apparent that his motor tics are causing issues for him in music - It is hard to keep in rhythm, do the hand motions to the songs, etc when you have to tic so frequently. The rest of his teachers gave him above average grades. I have talked with our principal and she is willing to sit down and discuss a 504. Earlier this year when I mentioned it, I was told by the school nurse that he would need one for middle school but that it wasn't necessary for elementary. His main issues at the moment are motor/vocal tics, "fidgetiness", and anxiety. He has an IHP (Individualized Health Plan) in place at the school that covers his celiac's disease, pancreatic insufficiency, and notification to me of Strep in the school.
harmony Posted December 11, 2009 Report Posted December 11, 2009 We get several warnings every year- I just call up the BOE and tell them I have Dr.s notes for every absence. They always say "no problem- we just have to send the letters out anyway". My son has missed around 15 days already this syear- not to mention early dismissals and tardies. If we decide to give my son the invasive sinus surgery that his Dr. suggested- we will just put him on hospital homebound. How many days does he miss a year on average? The note that gets sent out in Ga.- is a joke- the BOE will be able to tell you exact number of absences is allowed.

The school nurse is not qualified to make this determination. Utterly ridiculous!
mama2alex Posted December 11, 2009 Report Posted December 11, 2009 I don't know, but I would love to see one. Thanks for posting this request.
tantrums Posted December 12, 2009 Report Posted December 12, 2009 I was hoping there WAS one in this topic Things may have changed, but I worked for CPS a while ago. At the time I had several cases of kids who had missed a lot of school and they were called in as neglect referrals. All three that I had turned into nothing because there was no such thing as educational neglect. Being the dumb childless 22 year old that I was (and I have since decided no one who does not have kids should be allowed to do that job! I was SOOO clueless!), I actually told the one school principle who was really pushing it, that they should file charges on the mom. That child was about 7 and had asthma problems. Again, being clueless I felt it was the parents responsibility to get the child to school at that age. I was told by three different elementary schools that they do not persue truancy charges at the elementary school level, only at middle and high school. I felt that was silly since wouldn't parents have less control over it when the kids are older? But again - that was 15 years ago and in NJ. States may be different and in 15 years laws may have changed. The one case that I referenced though - the child had missed like 75% of the school year. And what I learned was there was NO legal ramification at all. But yes, the school sent notices every day. And since I mentioned the CPS aspect of it - keep your medical records parents just in case you get some school official that tries to go that route on you
thereishope Posted December 12, 2009 Report Posted December 12, 2009 I thought this was an interesting website... http://specialchildren.about.com/od/specia...acherpacket.htm Granted, in their list of disorders, PANDAS is not listed, but you can get ideas from clicking on the individual disorders that are appearing as a result of your child's PANDAS (OCD,ODD, TS, etc).
