What to expect after starting antibiotics?

Hi, I am new to this site and live in Australia where there is very little knowledge and recognition of PANDAS. Yet, my 7yo son is suspected to have PANDAS after a recurrence of tics 9 months ago, that got drastically worse 2 months ago at the same time as tonsillitis, but got better when taking ibuprofen, and worse again when stopping ibuprofen. He's now showing increased hyperactivity, elevated ASO, and bedwetting (the latest symptom). He started azithromycin 2 weeks ago and the bedwetting nearly completely stopped (just a minor accident last night), but the tics and hyperactivity are improved some days and as bad the next. I just don't know what to expect in terms of improvements (I heard it's a bumpy road!), is up and down the normal? Also, doctor said not to use NSAIDS for more than 5 days, but I believe some here use it for much longer?

Thank you for your help, it is really a struggle to get any support here in Australia for PANS/PANDAS.
