1tiredmama Posted July 28, 2012 Report Posted July 28, 2012 (edited) I got a call from the doctor's office saying that dd (recently dx'd PANDAS) has an active myco infection and that the doctor ordered Zith and PEX. I have not seen the lab results, so I don't know any more than that. Dd is currently on long-term Augmentin. I asked if Zith is to replace Augmentin or be in addition to. The staff did not know. I haven't gotten an answer, yet, nor has the pharmacy received the order. Here are my questions: 1. Will Augmentin alone clear it, if I never get the Zith? 2. How can dd have NO symptoms? 3. Can it cause sudden, severe asthma? I ask about asthma because dd never had asthma. Then, in December 2010, she got a respiratory infection and horrible asthma. She could not walk from one room to the next without an attack. It kept her out of school for two weeks, I'd never seen asthma so bad. It took a high dose of oral steroids and an abx to stop it. She had two more bouts that winter, a week each. Her ped said she had a virus both times. Since then she has been on Flovent daily from Nov-April with success. Last August, she had bronchitis. (Maybe dd had/does have myco symptoms.) 4. Could she have gotten myco in 2010? 5. Could it have hung on, despite 10-day courses of abx? 6. Could it be making her symptomatic periodically? Thank you! minimaxwell Posted July 28, 2012 Report Posted July 28, 2012 I do not think Aigmentin will clear it. Dr t thinks MycoP is my dd's trigger. I remember he added Doxy to the Augmentin. Yesterday we were at the ped b/c DD has fever and cough,I am testing her for MycoP (along w/many other things per dr T) Ped wanted to add Azith but her tummy can't handle it. He gave her 100mg Doxy BlD
PowPow Posted July 28, 2012 Report Posted July 28, 2012 My daughter was on Zith daily (250 mg) for a full size 13 yo- too low- went to USF over 4 months ago, Dr M and co. found active myco also- quite high. doubled Zith. They also added augmentin 1000xr BID (they were planning that anyway before the myco P labs came back). She is doing GREAT! You def need the zith. I would keep her on the Augmentin and get the Zithromax going= you can stop Augmentin if necessary when/if you hear back. Pex was helpful to my children- if you have any questions feel free to PM me. So you answer to #1 is NO #2: wish I knew! #3-6: YES! Mycoplasma airway infection can set off a cascade of airway inflammation that could create hyper-responsiveness (read: asthma)
DebC Posted July 28, 2012 Report Posted July 28, 2012 Our daughter was on a prophylactic dose of azith and when we had bloodwork run in May, her mycoplasma titer was 727 (normal should be 0 to 90). Our doctor increased her azithromycin to 250 mg every day and we are seeing improvements. She has been on this dose since mid May and will be on it indefinitely. She is scheduled for bloodwork to follow up on the titers in September. That bloodwork will probably determine how much longer we are on the full dose before going back to the maintenance prophylactic dose. She was completely asymptomatic for mycoplasma. She had no cough, no cold symptoms, absolutely nothing so I was amazed when her titers were high for it. She had been battling a return of mild to medium PANDAS symptoms for 5 months and we couldn't figure out why. Once we started the azith at a higher dose and added GABA (to battle her extremely high glutamate levels), CoQ10 (as she was extremely low on this enzyme), and zinc (as she was also deficient there), we have seen great improvements. She still has some residual anxiety, but it is gradually getting better. I don't believe that augmentin is effective in treating mycoplasma. I also believe that a course of antibiotics longer than 10 days is needed in PANDAS kids and should be followed up with bloodwork to determine i the infection has truly cleared.
