You should really take it especially if you are struggling.

ITA with this.. you might not even need an IEP just the 504. It's there for your protection and noone will need to know you even have it except the school and your teachers.

My dd is only 8 and in 3rd grade We just found out what was going on was PANDAS back in May 2010, but she has been having issues from it since January 2009. During that time not only did she have Mono and strep, but alot of the PANDAS symptoms on and off making it where she didnt' make it to school.. alot of the time she had the doctor slip because she was sick sick.. but on those days she was just an emotional mess and couldn't make it to school I kept her home but with out the doc. slip.. in her 1st grade year she missed 30 days of school(that was the Mono year)and all but 5 of them had a doc slip(though the doc for a whiel had a slip saying she could come and go depending on how she was feeling from the mono like a blanket slip covering alot of days) The next year while we were trying to figure out what was going on with her she missed over 20 days of school. So this year we got a 504 in place for her now that we know it's PANDAS we know there are going to be days where she is having symptoms and may or may not be able to attend school, and or she might need extra time to complete her work or somethign like that if something is messing with her brain keeping her from being able to do it..

She is only a 3rd grader but has always been a straight A student and not a problem child at the school at all, So at 1st when I went to them about needing a 504 they thought I was crazy, what does a perfect student need a 504 for.. but I didn't want to have to run her to the doctor on symptom days to get that doc slip every time.. and I also wanted to be notified when there was strep at her building so if there was a big out break of strep like strep in every class and every grade, I would beable to pull her out of school let her do her work from home sort of to try to protect her from the strep, without those days counting agaist her and without it damageing her grades.

I had the doctor send me in a few papers, one stating she has PANDAS and due to that she may need some accomadations, and the second one the school just wanted to see that a doctor agreed so I had him write me up something stating that it would be helpful for my dd if there was a huge amount of strep in the building for her not to be there.

The school after I gave them all the info I could on what PANDAS is and what it does and those notes from her doctor has been great working with us.. I go in next week to sign the final copy of the 504 and then she will have it legally in place to protect her which is wonderful Seeing how she has already missed 5 days of school over the last 2 months because of doctors appt and blood work and 2 days where she was having symptom that made it impossable for her to get to school.. To be on the safe side I got a doc slip for each of the days she has missed so far even symptoms day.. well I guess the 1st symptom day I didn't call her doctor but the 2nd one I did and he ordered some blood work that day and wrote a slip excussing her from school due to testing for us that day.

I guess if your school counts doctor excussed days agaist you.. a 504 plan really seems like what you need to do..
