MYCOPLASMA! could this be the key to some cases?

Dear Parents,

 

I have just encountered two cases of acute sneezing tics in which there was acute (markedly elevated IgM) Mycoplasma infection plus possible strep co-infection. Mycoplasma responds to zithromax specifically and not other antibiotics ...as well as IVIG. So I'm certainly considering this in the differential diagnosis of PANDAS-like illnesses.

 

I wonder if there is some sort of interaction between Mycoplasma and Strep at the immunological level ....

 

Have any of your children been tested for this ....

I believe that my Dad has had some real issues with mycoplasma. You know a bit about him.

My son tested positive for strep and below are the results for M.PNEUMONIAE. All the explanation I got was that he may have had walking pneumonia, is this correct?

 

The test results reads as follows.

 

M. PNEUMONIAE AB (IGG, M) , EIA

M. PNEUMONIAE AB (IGM) = 61 (Reference range <770 U/mL)

 

M. PNEUMONIAE AB (IGG), EIA = 1.23 (Reference Range <= 0.90 ISR)

The note reads: a positive result indicates that the patient has anitbody to Mycoplasma. It does not differentiate between an active or past infection. The clinical diagnosis must be interepreted with the clinical signs and symptoms of the patient.

 

Is this something I need to be concerned about? He has been on different antibiotics and his strep antibodies are coming down, although I have not seen a sudden decrease in symptoms just with abx. He has never been on Zithromax. Currently he is on Augmentin.

what type doc did these tests? was this part of immuno tests? the doctor didn't think the positive to mycoplama antibody was significant?

did you ever think your child DID have walking pneumonia at any time? I think this is where they are sick with flue or bronchitis or somethng, but it is actually pneumonia and you don't know it yet or it goes undiagnosed as pneumonia. sometimes you get or stay sick until it is dxd. I had that as an adult many year back, and so did my husband, but my point is that eventually it was realized that it was pneumo (walking pneumonia), guess that refers to that your walking around with it and don't know it.... :)

I am not sure how to answer this questions... If I understood right, the first results are the ones for "active" pneumonia, and the second just means he has the antibody. I never did think he had pneumonia, he has never been treated for it, and I know Augmentin wouldn't deal with it. The doctor who first diagnosed him with PANDAS ordered all those tests, along with many, many others, and went through the results with a fine tooth comb, so since she didn't make an issue of it, I thought it was nothing to worry about.

 

But, I now wonder if there is something to it... he has been on Amoxicillin, Cypro, Ceforoxime, Vancomycin, and lately on Augmentin. He is doing better slowly and steadily, but having setbacks everytime time anyone gets sick around him... I suppose that is part of this PANDAS thing...

 

I wish there was ONE doctor who could solve this for us! Dr. Latimer is treating him for PANDAS and I believe she is on the right track, but we still have to figure out what is going on with the family as a whole, and what's up with my 7-yo dd. Dr. Latimer refered us to an infectious disease specialist, but at this point we will need to wait until after baby is born (no news yet!). I also would like to consult and immunologist, and have everyone looked at, I just have to try to find someone in our network. The pediatricians are being cooperative, but I don't get the feeling they really know what they are doing or are willing to go very deep.

 

Anyway, I think I might at some point do a consultation with Dr. T to make sure we are covering all the basis. I know we are going to get to the bottom of this, I just sometimes feel like I am running around like a headless chicken trying to pursue too many leads. The first doctor we saw was good but too much into alternative stuff and not really addressing the autoimmune nature of PANDAS (never mentioned steroids or IVIG), just kept adding supplement upon supplement. Dr. Latimer is addressing the neurological part but not really looking at his overall health (which is fine, her specialty is Neurology, I am not criticizing her in that respect). I just wish there was ONE doctor (nearby and in-network, pretty please?) who could deal with the whole thing.

 

I am going on an on here... better get back to nesting :)

Sandra (to be 20 in a few weeks) got suddenly ill at 15 after Influenza A and Mycoplasma Pneumoniae.

Antibodies have been positive, rising ever since and fluctuating strongly in the 4-5 times the positive level.

Reacted well to Zeclar and Prednison 60mg clear for one year. Relapse following resp. infection. Same treatment yielded no result. 60mg Prednison with long term tapering no results. reacted well to IVIG in May 2007. 4 clear months. Maintenance schedule not maintaned and an ear infection triggered again. repeated IVIG and 1 pusle of Medrol for 3 days did good but nto continued (strange story). Risperdal had terribel effect. No immunomod. treatment since december 2007 as no specialist in Finland interested. Situation deteriorated albeit intensive cognitive work. Started Zithromax long term 2 days ago. Sent you her MycopP values via private message. The brave GP treating ehr will love to have your support and assistance and so do we.

We must save Sandra!
