Question about antibiotic dose PANDAS and active strep. infection

Hello!

 

New to this board.

 

My daughter has PANDAS and was just diagnosed with strep yesterday. I went to her regular doctor and was given the standard dose of antibiotics and I would like to know the opinions on this board of what dose and length of treatment is recommended for PANDAS kids.

 

Also, my niece and nephew have PANDAS. Any opinions on how long to keep my daughter away from them to prevent a reaction to her strep?

Hello. I would say to keep him on abx until you get a negative strep test for one. Also, if she is still symptomatic, (pandas symptoms), I would stay the course of abx until you see a remission on symptoms. My son, 2 years since Pandas onset with strep, reacts differently to abx. The window of time in which it takes the abx to bring about a remission is different with each episode. I have seen him go back to baseline behavior in 2 days, (mild episode), and other times it has taken 6 weeks, 3 rounds of abx. His average response time is 2 rounds of abx.

 

I would keep her away from cousins until she has a negative strep test.

She isn't having an episode right now, but I am bracing for the one that might follow this infection. I had hear that possibly the actual dose of the abx should be higher for PANDAS kids. Anyone know anything about this?

What abx. did they prescribe? What is the age and weight of your daughter? In my opinion amoxicillin is too weak of an antibiotic to be given to pandas kids ever. Augmentin is much better if she can tolerate penicillins. Again, for an active strep infection it should be a pretty high dose for her age and weight. I think most of the pandas experts agree that 30 to 60 days of this treatment dose is a good place to start. Once the active infection is gone then dose based on symptoms. If symptoms are gone and you are back to baseline then a prophylactic dose should be considered, especially when strep is spreading in the community.

If you know she has PANDAS you should start looking for a PANDAS specialist right away. You will need a minimum of one month of antibiotics (3 is best). Most likely you are only going to get this from a PANDAS specialist. If you can't get any help, go to the NIMH website and you can e-mail Dr. Sue Swedo. One of her collegues will contact your physician and give them the latest update on how to treat PANDAS. You will need to give information on your childs medical condition and how things started in addition to your pediatricians contact information. This has worked well for several people I have talked to.

My daughter for the first 2 years was very mild, didn't need long term prophylactic abx, and whenever she got strep we always did a 30 day course of abx minimum. We always did 20 days minimum when she would get strep prior to her PANDAS onset b/c her brother had PANDAS, and our ped didn't want to take any chances (it's the reason I think she stayed mild for so long...until a particularly nasty strain last year.)
