Some kids (particularly PANDAS kids) can become activated (i.e., display worse behaviors, rather than improved ones) on even conventionally low doses of SSRI's. It's a hard concept for some psychs, though, because they tend to think that if a dose is below the maximum level indicated for a given condition/age/weight, then it should either just 1) work or 2) not work in terms of lessening anxiety, but they don't expect or understand the activation, so sometimes they don't believe it. I suggest getting some of the behavior on video . . . smart phones work great for this . . . and then show it to your prescribing doc.
