How could my pandas son catch strep while on abx?

My son has Pandas and has been on abx for over 18 mos straight. He is having a bad sore throat and it has white spots on it. I am assuming it is strep and have called the doctor and am waiting for a call back. How is this possible? Where do we go from here?

 

Culture first to make sure it is strep.

 

How much does he weigh, and what dose and type of antibiotic is he on?

 

There are Azith. resistant strains of strep on the East Coast.

 

IMHO, Amoxicillin/Augmentin 1x daily isn't effective for PANDAS kids.

 

Also, IMHO, the dose (of whatever ab you are on) is important.

He weighs about 130 lbs and is on 500 MG Cefaclor 2x a day.

 

We saw an increase in his tics in the past 2 mos and we're waiting for an appt in early July with an ENT to consider a tonsillectomy.

 

I'm not seeing an increase right now in Pandas symptoms other then a slightly increased frequency in tics which has been going on for the past two months.

 

I'm scared.

Get to a PANDAS doc ASAP. Peds rarely have the specialized education to comprehend much outside of the box, in my opinion. Do NOT wait. It is at least a 2 month wait list to get in. You will likley have to travel to get there. It is worth it. I do not see that you stated your location. Big gun specialists on the east Coats and the midwest that i am familiar with: Dr. Bouboulis, CT; Dr. Trifiletti, NJ; Dr. Latimer, Wa, DC; Dr. Murphy, FL; Dr. Kovocevic, IL; These are the docs that I would consider close enough to travel to for help, and I live in NC. We see Dr. Bouboulis in CT.

This also happened to my son who is diagnosed and just recently to my daughter who hasn't been diagnosed (but she was taking treatment level abx for Bartonella). It can happen if your son has no antibodies for strep. My son had his Streptococcal Antibody titers run and he has no antibodies for strep.

 

Is your child seeing an immunologist? I would highly recommend getting to one ASAP for further testing and treatment dose meds for throat.
