reading errors

I have an average reading rate of 200-240 wpm but make several errors while reading, such as skipping or misreading words, eg. 'that' for 'than', 'could' for 'would'. Sometimes, I sound a word in my head that is entirely different from the one on the page. These errors occur 5-10 times per page. I am really frustrated but no matter how carefully I try to read, I still make mistakes.
Is there something wrong with me?
